Corrected in Form 1094-C
If you’re correcting information on the Authoritative Transmittal Form (identified on Part I, line 19 of 1094-C), file a single, fully-completed Form 1094-C including all the correct information. Enter an “X” in the CORRECTED checkbox. There’s no need to file a corrected 1094-C form for any previously filed 1094-C that was not the Authoritative Transmittal.
